Every summary opens with the patient’s demographics, the source file and a completeness statement — every page processed and accounted for. The record overview covers chronic conditions, surgical and structural diagnoses, screening history, vaccinations and documented allergies.
Every clinical term coded and traceable to its page
The index lists each clinical term with its category, first-recorded date and the exact source pages it appears on — so any code can be traced back to the original document in seconds. NovaHS staff review each item, and the relevant SNOMED codes are entered directly into EMIS or SystmOne.
Wrong-patient detection and a full audit trail
NovaSummarise automatically flags possible wrong-patient pages — for example where an NHS number conflicts with the record identity. Flagged pages are excluded from the summary, logged with a full audit trail, and exported as a separate PDF so they can be refiled against the correct patient.
