A GP practice in Hereford required additional clinical support to manage patients with hypertension, ensuring regular reviews and treatment optimisation without adding further pressure to its existing clinical team.
Our solution
NovaHS introduced dedicated hypertension clinics three times a week. Advanced Nurse Practitioners provide ongoing patient reviews, blood pressure monitoring and medication optimisation, adjusting medication where appropriate and arranging follow-up to support better blood pressure control.
The outcome
The practice benefits from consistent clinical support, timely reviews, appropriate treatment optimisation and ongoing follow-up. The service provides additional clinical capacity and reduces pressure on the existing practice team while supporting long-term hypertension management.
The clinics support the practice in working towards QOF targets relating to blood pressure monitoring and control. No measured change in blood pressure control or QOF achievement was supplied for this case.
